This form is the starting point from which we will begin your appointment process.

Please complete all pertinent areas as accurately and concisely as possible in order to process your idea and schedule your appointment as easy as possible. Once we have gathered all the pertinent details of your tattoo idea, we will determine an appropriate amount of time for your appointment. When the consultation process is completed and we have given you the go-ahead to book an appointment, we will ask you to call us to set up your date and time with a $100-$250 non-refundable deposit.
